Toppings stored in containers with single-serve ice cream cups used as scoops. Walk-in cooler with condensate leaking from seam above rack in back of cooler. Two of the three soft-serve machines have condensate leaking onto tops of mix containers. Employees handling food without hair restraint. Can opener blade with dried food debris. Several utensils (knives) in container at ice cream cake area with food residue on them. Mixer shafts not sanitized every four hours as required.

Floors, walls and ceilings have a buildup of food debris, mold/mildew and grease. Hoods have heavy accumulation of grease and dust.

Hand sink in bar not stocked. Cutting boards have an accumulation of mold/mildew. Sauces served using a plastic to-go cup. Repeat violation: floors have food debris and mold/mildew. Sinks are no longer attached to walls and walls have mold/mildew.

No paper towels available at hand-washing sink beside veggie prep sink. Floors have food debris, crumbs, etc. under shelving and equipment. Several tiles along fool/wall juncture are loose and need to be resealed or replaced. Accumulation of dead bugs found along base of floors and in bottoms of bins and containers.

Can opener has a thick buildup of food debris and grime. Kitchen ware washing sink has wash and rinse setup but no sanitizer compartment setup. Dishwasher is not dispensing any sanitizer. Utensils hanging on drying rack with food debris and sticky residue. Pans stacked wet nested with food residue and slime on the insides of pans. No consumer advisory posted. Several containers of food prepped and not date-labeled. Raw eggs and bacon stored above ready to eat foods in reach-in coolers. Spoiled lemons stored with good lemons in walk-in cooler. Tape used on handle to reach-in freezer. Inside and outside of microwave dirty with dried food debris. Sides, racks, bottoms and gaskets in cooler have food residue. Food debris splattered on lower portions of wall. Hood vents and box fans in kitchen have an accumulation of dust and grease.

Floor in front of cook area has a section that was dug up for a repair and the floor is not durable, smooth or easily cleanable.

Repeat violation: Grease container covered with grease and is leaking and dripping grease all over Dumpster area.

Walk-in cooler evaporator also has condensate leak which is being caught in a plastic pan. Floor in walk-in cooler needs cleaning at floor/wall junctures. Fluorescent bulbs in tubes over hand sink/mop sink/storage area without end caps on left side of bulbs. Liquids from prep on table top above dribbling into sugar container because lid is not always closed tightly between uses.

Walk-in cooler not holding potentially hazardous foods at required temperatures.

Box of sausage and containers of syrup sitting on floor in kitchen. Single-service cups and lids stored on floor. Chlorine test strips missing. Tops damaged on dumpster. Doors of Dumpster open and Dumpster leaking grease/food on ground.
