Leftover chili reheated in microwave for hot holding in steam unit with temperature of 107 F. Leftover potentially hazardous foods to be reheated for hot holding must be reheated to 165 F within two hours before hot holding. Chili cooling in full 6-inch small hotel pans covered in plastic wrap with small hole in each plastic cover in the walk-in cooler struggling to cool as required. Ice machine was not working to provide ice for ice bath. Sheet pan of cooked mushrooms and onions and peppers was cooling on prep table until cool enough to handle for portioning. This method allows food to cool only to room temperature and portioning does not take place sometimes for several hours after cooking; start to cool in walk-in cooler with sheet pans loosely covered after cooking and until time allows for portioning. Water heater not working and emergency tank installed several days ago not providing hot water to the facility. Dumpster area with trash piled between dumpsters and between them and walls of enclosure, as well as trash items pinned under dumpster. To prevent insect and rodent attraction, the dumpster must be kept free of trash. They need to be moved out of enclosure to allow for thorough cleaning of the area. Half of hinged lid is missing from right dumpster in enclosure. To prevent insect and rodent attraction, it must be able to be closed up.

Items that are kept longer than 24 hours must be marked to be discarded within seven days, including the day of preparation. All items had been cooked on Saturday and Sunday. Tongs were stored on pipe (exposed to contamination) below stove top; store on a cleanable surface. Plastic container was used as a scoop in flour bin. Utensils used as scoops must have handles.
