Foundation Food Group faces more than $500,000 in potential fines related to a Jan. 28 nitrogen leak that killed six people, the Occupational Safety and Health Administration announced, calling the deaths “entirely avoidable.”

Floor fans with dust. Trash in ladies restroom needs cover. Walls, ceilings with food splatter need to be cleaned. Couple of places in ceiling where ceiling sheetrock is peeling and needs replacing. Replace last remaining two black end caps on light above grill area. Replace broken light shield on cold bar and all bulbs at hot bar must have a shatter-resistant coating on bulb.
