There is no certified food safety manager on staff. Manager wasn’t able to answer questions regarding temperatures; meat, poultry and fish should be cooked to kill pathogens (items checked were correct temperatures). Employees hadn’t been instructed on glove use, hair restraint, type of cups for drinks in the kitchen, how often to clean tea urns and product rotation. Manager prepping calamari with coffee in mug on prep table. Gatorade bottle on overflow tray at expo station drink dispenser. Disposable cup without lid and straw at end of pass-thru widow in cook area. Disposable cup with lid torn for drinking access on prep table in cook area. Tea urn nozzles and shafts of urns with large buildup of mold. Cooked rice wrapped and plated in walk-in cooler with date of preparation 10 days previous (March 18). Portion cups of slaw uncovered on trays with trays stacked on top of each other where bottom of tray contacts slaw in cups. Manager prepping calamari without hair restraint, cooks without hair restraint, dishwasher without hair restraint. Manager wearing watch, dishwasher wearing bracelets, server prepping salads wearing bracelets. Cloths used to wipe food-contact surfaces laying out of prepared sanitizer at cook line and prep areas. Manager touching face and then starting to help cooks plate food without changing gloves (stopped by inspector). Dishwasher handling clean dishes after loading soiled dishes with same pair of gloves on.

No gloves being worn after cooking gorditas and stuffing with cooked meat. Unpackaged food (i.e. bakery breads) on truck in container. Food employee was handling with paper towel to give to customers; however, all food must be wrapped prior to placing on truck. Hot beef (cooked) in container on truck. Meat placed back in restaurant to be placed in gorditas then wrapped. No food shall be prepared on the truck as there is no protection from environmental elements and no hand sink.
