Concentration of sanitizer (quats) not manufacturer recommendation. Cup for onions stored in onions and water. Food employees did not discard gloves after preparing raw hamburger; touching other equipment, and touching ready-to-eat food. Hot water at hand sinks not temping at required temperature of 100 degrees (temped at 75-76 degrees).
