Reach-in cooler (for fish) door left open, elevating food temperatures to 46 degrees and not 41 degrees or less for cold holding. Baked potatoes in small cooler, in food prep area, temping at 63-67 degrees after cooling more than six hours, per staff. Ice scoop at drive-thru had handle in ice. Pans and containers stacked wet after sanitizing, on dry storage rack. Standing cooler door will not close. A wire has to be looped onto screws. Coke machine at drive-thru has paint chipped above ice lid. Metal beneath is rusting, corroding. Old walk-in cooler used as storage has floor threshold coming up, and steel wall panel at bottom corroding and falling apart near coving. Grout missing in between kitchen floor tiles, allowing accumulation of water and food. Grease from tool, near fryers, is stored above electrical outlet, allowing grease to accumulate near plug-in. Light shield sheath missing at back prep table that has can opener.
