Scale has damaged protective covering for numbers, buttons capturing food. Racks in coolers had food debris, mold, mildew debris. Outside of hot hold warmer, on handle, gaskets in coolers, interior near doors of coolers has food debris.

Make line prep cooler with condensate leak (cup on floor catching water), and gaskets damaged on bottom of doors. Floors throughout kitchen and underneath equipment with food, grease residue. Walls at three-compartment sink and prep area with debris.

Inspection report from last inspection date July 5, 2013, with score of 75 wasn’t posted for public viewing. Condiments and straws kept in containers, cubbies with food debris. Single-service cups in wrappers placed on floor while restocking. Ice cream cooler gaskets damaged. Cooler shelving, outside of equipment, cooler gaskets, interior of coolers near drive-thru have food debris. Floors damaged in kitchen and near back door. Floors had food debris, trash beneath dry storage racks, in floor drains. Light shield near grill cracked. Entry doors have large gap allowing insect entry, and one not closing properly. Flies in kitchen, and need to have numbers reduced by obtaining control devices.

Corn, cheese not at less than or equal to 41 degrees in glass cooler near salad bar. Pasta from this morning cooling at 44 and 50 from 11:30 a.m., iced down to cool to 41 within six hours and moved to line cooler. Corn, yesterday’s prep, cheese put in cooler today. Clean dusty fan guards in walk-in cooler and shelves near window, where pizza prep occurs (food has dropped).

Vegetable sink filled with container, cardboard, single-service items, poster and lotion. Containers stored on clean storage rack has lettuce on them. Single-service items stored in vegetable sink. Vegetable sink plumbing below damaged and broken from bottom of sink.

Lids, pan and plastic pan holding lids not clean to sight and touch. Bottom shelf clean dish storage. Lids have accumulated debris and need to be cleaned before use. Chopper with onions left in blades stored cleaned. Three-compartment sink set up for utensils without sanitizer (hot water only) in third sink. Label sugar, yeast, bakery area and seasonings in dry storage with names since removed from original containers.

Score: 81, Grade: B

Whopper burgers not cooked to 155 degrees from flame grill. Prep cooler at front not holding butter blend at less than or equal to 41 degrees. Milk is ultra pasteurized and moved to walk-in cooler (unopened). Cheese at salad prep cooler at 50 degrees. Top opened with several pans missing allowing cold air to escape. Ice scoop improperly stored with handle in ice.

Score: 68, Grade: U

Current certified food safety certificate is expired and needs to have certification renewed. Demonstration of proper food safety not being properly displayed by staff due to severity of violations. Raw shrimp stored below raw chicken in walk-in cooler. Several products (beef, shrimp, etc.) stored below chicken. Bowl at self-service bar had trash in bowl. Coke diffusers had mold, mildew. Sanitizer solution at dishwasher 10 ppm per test strips (chlorine). Items on bar (hot-hold service bar) not temping at 135 degrees or more, (imitation crab meat at 110 degrees, fried fish at 109 degrees, chicken tenders at 117 degrees, pot stickers at 108 degrees) for hot holding. Mushrooms cut prior to washing and placed back in delivery box. Racks in walk-in cooker and other coolers are rusty and paint peeled off. Mold, mildew on racks of walk-in cooler. All lights at hot-hold bar and cold-holding bar are not shatterproof.

Dishes not rinsed correctly at three-compartment sink.
