Heavy mold, mildew on coke nozzles for self-service. Small two-door cooler by hand sink not keeping food at 41 degrees or below. Bacon (raw), lasagna, ham and cut tomatoes temped at 50 and 46 degrees. Lasagna, noodles and chicken wings in coolers not date-marked. Chess freezer handle missing and holes in freezer; interior exposed. Prep table in warewash area collapsing. Three-compartment warewash sinks growing mold, mildew and has food residue. Shelves, under tables, equipment in warewash area covered in food residue. Under shelves of cashier area has food residue and debris. Floors beneath coolers and tables dirty, caked with black debris and have food beneath. Floor tiles throughout kitchen and in mop sink room are broken, missing and needs to be replaced. Ceiling tiles are peeling heavily or damaged. Work surface made of particle board by sink is collapsing. Lighting intensity not sufficient in prep areas or warewash area.

Walls at hand sink, prep sink and walls in walk-in cooler with food residue. Ceilings in ladies’ restroom with a hole in the ceiling, exposing the wood above. Sheetrock peeling and pulling apart in some areas. Ceilings in men’s restroom with sheetrock peeling, damaged in areas. Ceiling tiles in both bars damaged. Floor tiles throughout entire kitchen damaged; grease on floor, underneath equipment. Floor in bar, next to stage is peeling paint. Ceiling in walk-in cooler peeling paint and few areas are moldy.

Leftover pulled pork reheated for stem unit, but temperature of pork ranged from 108-131 degrees. Barbeque chicken cooked in the morning for lunch stored in hot holding cabinet at 121 degrees. Cooked pork on counter to cool for 90 minutes at 111 degrees. Floors in kitchen need a good scrubbing. Tiles with ground in dirt and debris, under three-compartment sink and drying rack and cook line equipment. Ceiling panel missing in kitchen; over area where the bread is stored.
