Raw and ready to eat foods store all mixed together in walk in cooler. Shelves, evaporator unit in walk-in cooler with heavy dust accumulation. Can rack in kitchen with heavy dust at bottom, with dust accumulated on cans. Raw concrete showing underneath grill area; hole in wall behind floor mixer; floors in walk in freezer with debris.
