No paper towels at hand sink. Hand sink greasy, with food debris. Mold, mildew on interior ice machine surfaces and on dispenser heads of most pieces of equipment. Chicken nuggets dated March 27 at 60 F in reach-in. Employees cooking food without hair restraint. Dirty cloths, scrubbers, spray bottles in vegetable sink. All single-serve condiments (salt, pepper, mints) and single-serve articles (cutlery) are stored in containers with food debris, grease, etc. Most sinks are no longer attached to walls. Gaskets on several coolers are damaged. Gaskets throughout and nearly all equipment have grease, food debris. Floors, walls, ceilings need much better cleaning. Light in freezer not functioning properly.
