Score: 91, Grade: A
Scoops, tongs, knife (blade) ring of slicer with dried food debris. Too little sanitizer solution concentration in cloth storage bucket to be effective at sanitizing at serving line. Also, not enough actual solution to allow for all the cloths to be submerged in solution between uses. Ice scoop laying on towel on top of ice machine. Ice scoop on a towel on top of ice machine under Ansul canister that is dusty. Dumpster No. 2 rusted through on bottom front and leaking liquid onto concrete pad. Replace to prevent leaking that can be attraction for insects, rodents. Painted concrete block walls around mop sink with peeling paint - wall surface must be durable, cleanable and nonabsorbent.

Mold buildup on ice dispenser at Coke self-service machine, bread pans with bread pieces left inside pans and back on shelf. Inside of sandwich prep cooler with metal surface peeling off bottom exposing insulation underneath. Floors and wall beside ice machine with dust and debris. Coved baseboard separating form wall all along back wall underneath three-compartment sink. Floors in walk-in freezer dirty and seams pulling apart. Personal items (jackets, computer etc) on shelves with items for customer.

Slicer with food residue underneath blade, behind cover, on blade; scoop with food residue. Walls with mold in dish machine room. Clean walls and regrout floor tiles where grout is coming out. Floors need cleaning in dry storage room.

Score: 93, Grade: A
Ice machine baffle with visible mold growth (pink slime). Clean and sanitize baffle frequently to prevent growth. Wrapped breakfast items panned for tomorrow's breakfast being dripped on while stored under walk-in freezer evaporator during freezer's defrost cycle.

Score: 56, Grade: U
Cook washed gloves off in sink while wearing them. Food employee had drink in non-disposable cup above drink boxes. Rolls stored in Walmart bags. Soups and sauces stored in never-been-used work buckets. Squid not covered in cooler. Slicer had dried food debris on blade and on slicer guard. Several coolers not keeping food at required cold holding temperature of 41 F. Steak at 43 to 46 F, steak at 43 F, feta cheese at 43 F, and horse radish at 43 F. Check food regularly to ensure less than equal to 41 F. Moved to walk-in cooler and temperature lowered. French fries cooked and put on top of oven in container at 88 F. Keep hot at 135 F or above once cooked or cook to order. Sanitizer in cloth buckets (chlorine) and soaking coke nozzles at bar exceeding 200 ppm per test strips. Coke nozzles recleaned. Wiping cloths stored on side of food employees aprons. Gloves not discarded between uses of preparing food. Dumpster missing drain plug. Grease trap container has grease on outside which has dripped to ground. Clean and properly dispose of soiled debris on ground. Light shields cracked and sagging above coolers in kitchen.

Score: 93, Grade: A
Condiments (salt, pepper, ketchup, etc.) stored in containers with food debris, etc. Three-compartment sink and hand sink no longer sealed to wall. Hot water at hand sinks below required 100 F: water temps at 95 F, 78 F, 83 F. Hand sink at drive-thru has a leak in drain. Grease dripping onto ground in dumpster enclosure from grease container. Cove molding in walk-in cooler and by cook area no longer attached to wall; some chipped tiles in high traffic areas; observed grout missing in high traffic areas.
